If you've lost your vaccine card, or just need to access your vaccination history electronically, the Illinois Department of Public Health now offers an online tool called Vax Verify which lets you get your information quickly and securely. Read the directions below and then visit:

To access your record, you MUST use the name you provided at the moment of vaccination. Use of middle names, nicknames or multiple last names could affect the system's ability to find your record. If you've tried Vax Verify and still cannot locate your record, you can also try the Chicago Department of Public Health (ONLY for residents of Chicago) at:
