Scheduling an appointment can be done by calling our main number at 773.584.6200 or stopping by one of our three sites to speak with a Patient Service Representative. Below you will find tips on what to bring to your next visit, rescheduling, and information about our sliding fee scale. We are looking forward to serving you!
Preparing for the Visit
Our friendly medical records coordinators will be happy to call and remind you about your upcoming appointments. They will also encourage you to prepare for the visit by bringing the following items:
• Bring a list of medications you are currently taking
• If you were admitted to the hospital, please bring your discharge papers
• Bring your list of questions
Unable to keep your visit?
It is important that you keep track of all scheduled appointments but we realize that emergencies happen and realize that there will be occasions that you will need to reschedule your appointment. It would be helpful for us to know of your cancelation 24 hours in advance in order to offer your time slot to another patient. To reschedule an appointment, simply call our main number at 773.584.6200.
Need to be seen but you do not have medical insurance?
Since Esperanza Health Centers is a nonprofit organization we are able to seek private donations to help see many individuals in our service communities that are unable to afford the cost of purchasing medical insurance as well as those individuals that are not able to participate in the market place because of their status. Esperanza Health Centers has a sliding scale fee and many of our uninsured patients pay $30 to $50 for comprehensive healthcare services or a $1 to $5 fee for mental health counseling.
